Information is used to provide healthcare, treatment, payment and clinic operations, and to communicate with patients about appointments and administrative matters, as permitted by applicable law.

Disclosure of Information

Information is disclosed only as permitted or required by law, including to other healthcare providers involved in a patient's care, with appropriate consent.

Website Forms

Personal health information should only be submitted through the clinic's approved secure forms and channels. Do not send detailed medical information through the General Contact Form or ordinary email.

Your Rights

Patients have the right to access and request correction of their personal health information, subject to applicable legal requirements. To make a request, contact the clinic using the details on the Contact page.
