Open the New Patients page and complete the secure Patient Enrolment Form.

Appointments

Select Book an Appointment, choose an appointment category and select an available date and time.

To cancel or reschedule an appointment, use the cancellation or rescheduling option provided with your appointment confirmation. If you need administrative assistance, contact the clinic.

Bring your health card, medication list, allergy information and relevant records. Visit Preparing for Your Appointment for more guidance.

Services

Services include acute illness assessment, preventive care, health monitoring, medication review, test interpretation, referrals and coordinated care.

Yes. Supreme Medical Clinic provides comprehensive mental-health care within a family-medicine setting.

Virtual Care

No. A patient must complete the first visit in person before accessing virtual appointments.

To continue using virtual care, patients must attend at least one in-person appointment every 12 months.

No. Some concerns require an in-person examination, testing or additional assessment.

Medications and Prescriptions

No. The General Contact Form is for administrative communication only.

A clinical review, appointment or appropriate monitoring may be required.

No. Prescription and renewal decisions depend on appropriate clinical assessment.

Referrals

The Referral Form may be submitted by anyone assisting a patient's care, except the patient themselves.

Referrals are accepted for both Comprehensive Family Medicine and Comprehensive Mental Health Care.

Follow the fax or approved secure-submission instructions shown on the Referral Form page. Do not send referral documents through the General Contact Form.

Urgent and Emergency Care

Call 911 or go to the nearest emergency department when immediate medical assistance is required.

Call or text 9-8-8. Support is available across Canada 24 hours a day, seven days a week.

No. Website forms and online booking are not emergency services and may not be reviewed immediately.
